Welcome to the USA Stars Academy scholarship application questionnaire. We will try to accommodate your family's financial needs as much as possible.
All of your answers and documentation will remain private and not shared with the coaches, managers, or other players.
At the end of the application, we require you to upload income verification documentation. Examples are two sets of paystubs or your IRS form 1040 from 2020.
We will be asking all our scholarship families to assist with volunteer work throughout the year to offset these financial discounts being extended to your USA Star player
